How to send self-assessment questionnaires to your suppliers?

If a supplier has been marked at risk through the abstract risk assessment, one tool you can use is the self-assessment questionnaire (SAQ) to confirm or not the presumptions of risk.

Here is a video that describes how we structured our SAQ at Daato. You can send these questionnaires from the “Repository” tab.


There are two options: 

Option 1: Send the SAQs in bulk to all suppliers selected.
Click on “Review and Send” in the 2nd blue box. This will open a request window on the right with pre-selection of all the suppliers that are marked as high-risk.



Option 2: Send a SAQ individually
Click on the ⋮  icon for the supplier in question and select “Send self-assessment”. This will open a request window on the right.


 

For both option, the request window will contain the same field: the name of the request, the supplier(s) selected, the deadline to answer, the selection of human rights and environmental topics, and a message. 


Bear in mind that high-risk supplier will be required to fill in their self assessment for specific risks that were identified, but you also have the option to add additional topics.  


Once everything is filled out, you can click on “Send”.

 



What do the suppliers see?


The suppliers concerned receive an e-mail notification and can log in to the tool to answer the questionnaire. On Daato, the supplier then sees a questionnaire that they can answer step by step. The supplier can also forward it to colleagues for processing and send it back to you after completion by clicking on “Reply”.
